The Source:

Annett Stülzebach:
"Vampir- und Wiedergängererscheinungen aus volkskundlicher und archäologischer Sicht"
from: "Concilium medii aevi" 1 (1998)

The Case:

When archaeological research took place at an ancient cemetery near Brzesc Kujawski, the remains of a man were discovered. A wooden stake had been driven through his left chest and, apparently, through his heart.

The Date:

The grave is said to date back to the 2nd half of the 11th Century, possibly early 12th Century.

The Place:

Brzesc Kujawski can be found about 10 kilometer of Wloclawek, which in its turn can be found West of Warsaw.

Possible Follow-Up:

You could start with finding and checking out Annett Stülzebach's rendering of the facts. And after that you can move on to her source: Kaszewscy: "Wczesnosredniowieczne cmentarzysko w Brzesciu Kujawskim". Good luck, I do hope your Polish is better than mine ! : )
